Problem Statement

Every year, over 1.1 million medical examinations are required by government agencies such as MOM, ICA, SPF and LTA for statutory purposes , including foreign work passes (e.g. work permit & S Pass) vocational driving licenses and driving licenses for people above 65, issuance of student pass, Permanent Residency & Long Term Visit Pass.. Each of these examinations has different submission processes. Most of them are required to be submitted to Government agencies by individuals and corporates after collecting from clinics. Some are required to be submitted by clinics directly to agency but via different portal and FormSG links

This fragmented system causes workflow inefficiencies and submission errors, particularly:

• Confusion among clinics: Staff struggle to manage multiple submission portals, leading to inefficiencies in training and daily operations.

• High error rates: In 2024, clinics made 450 incorrect submissions for Migrant Domestic Worker (MDW) 6-Monthly Medical Exams due to confusion with Female Migrant Worker 6-Monthly Medical Exams

• Growing complexity: As more agencies move to digital submissions by clinics, clinics face increasing difficulty in managing diverse reporting requirements.

This inefficiency not only burdens clinics but also reinforces the public perception of government agencies as siloed and inefficient. It also results in wasted agency resources spent on correcting submission errors and following up with employers.

Objectives and Product Goals

We developed CheckUp, a centralized platform to streamline medical examination submissions, with the following goals:

• Improve Workflow Efficiency: Reduce administrative workload for both clinics and agencies.

  • Cut clinic submission time by 50%

  • Cut agency verification time by 100%

  • Reduce time spent following up on incorrect submissions with employers.

• Enhance Submission Accuracy: Reduce errors by 80% year-over-year, bringing incorrect submissions down from 450 to 90.

Key Features:

✅ Real-Time Status Dashboard – Clinics track pending examinations, eliminating the need for manual tracking.

✅ Seamless Agency Integration – Direct submission to agencies, reducing back-and-forth clarifications between various parties.

Research and Approach

To understand the pain points, we conducted:

• Field research with MOM Onboard Centre for Migrant Workers, 4 Migrant Worker Medical Centres by 4 different operators and 4 GP clinics, shadowing staff workflows.

• Stakeholder interviews with agencies including MOM Workpass Division and Assuance Care and Engagement Division LTA Point-to-Point Transport Division, and ICA Policy & Development Division.

• Workflow mapping to identify bottlenecks and inefficiencies.

Solution Overview

CheckUp is a web-based platform that simplifies medical examination submissions.

Key Features & Benefits:

• One-Stop Submission Portal: Eliminates the need to toggle between multiple government portals.

• Error Prevention System: Reduces incorrect submissions through pre-filled data and automated form checks.

• Live Submission Dashboard: Tracks pending and completed submissions, freeing up clinic assistants’ time.

• Agency System Integration: Ensures medical result data reaches the correct agency system without manual intervention, reduces the need for manual verification and enables straight-through processing.

Outcomes and Impact

The following medical exams are built in the prototype:

1. Full Medical Exam for Migrant Workers

2. 6-monthly Medical Exam for Female Migrant Worker

3. 6-monthly Medical Exam for Migrant Domestic Worker

4. Medical Exam for PR application

We have let clinic users try out Full Medical Exam for Migrant Workers and 6-monthly Medical Exam for Female Migrant Worker using dummy data.

Next Steps and Future Vision

To scale CheckUp, we aim to:

• Test out 6-monthly Medical Exam for Migrant Domestic Worker and Medical Exam for PR application with clinics

• Set up Agency API Integrations: Connect directly with MOM, ICA, and LTA systems for seamless data retrieval (for verification) and submission (for processing). As a start, to partner with MOM to

  • Roll out the new Full Medical Exam for Migrant Worker together with MOM's revamped Workpass Issuance process in Q4 2024.

  • Roll out the new 6-monthly Medical Exam for Female Migrant Worker and 6-monthly medical exam for Migrant Domestic Worker to achieve straight-through processing and minimise wrong submission in Q3 2024

• Pilot Program with Medical Centres: Test the platform on real case submissions.
